Adoption Application

As a volunteer-run organization, it takes time to review applications. We are looking for the best fit for each of our foster dogs. Please confirm both of the following statements:

I am applying for a dog that is listed as Adoptable

I understand that applications for dogs that are not listed as adoptable will be disregarded

As a volunteer-run organization, it takes time and resources to process applications, especially vet checks. We ask that you only apply if you know your vet check will pass and are sincerely interested in adopting a dog.

I give permission for a FSDR Representative to contact my vet reference to complete a vet check for my application.

Please confirm the following:

I agree to a FSDR Representative doing a background check to ensure I am a good candidate for adoption.

I agree to have a FSDR Representative visit my home prior to adoption.

The primary adopter and caretaker of the dog is 21 years of age or older

I confirm that I have discussed adopting a pet with everyone living in my home. Everyone is onboard and excited to adopt.
